How My Viva works
My Viva Plan® (MVP) automates the creation of comprehensive annual care plans (CACP) for your patients with complex needs. The plan is used by pharmacists, doctors and healthcare professionals or other digital solutions across Canada to refine the specific activities and actions being tracked and the ensuing feedback to the patient.
MVP addresses the labour-intensive process that pharmacists must undertake to establish an agreed plan of action with their patients. For sustained behaviour change to ensue, patients need to engage on a daily basis rather than simply tracking activities and clinical indicators (i.e. weight, blood pressure, blood glucose, etc.)
MVP is used for patients living with conditions such as obesity, pre-diabetes, diabetes, heart disease, musculoskeletal conditions, and other cardio-metabolic conditions, many of which are simultaneously related to mental health disorders such as depression, anxiety, and others.
Clinically-validated results
My Viva Plan® has had an economic evaluation completed by the Institute of Health Economics (IHE).
Economic evaluations in digital health are challenging as they need to be compared to economic evaluations with standard clinical practice which is fairly scarce. However, we collaboratively work with the CHANGE program from Metabolic Syndrome Canada and CHANGE Alberta who have built a standardized program for cardiometabolic rehab and have had an economic evaluation completed by the IHE. This work allowed the IHE to compare MVP to the CHANGE program.
The results are most compelling. IHE determined that alongside the CHANGE program, MVP can reduce the incidence of ischemic heart disease by 7 cases, Type 2 diabetes by 18 cases, and 1 less death per 1000 patients over 10 years. In addition, MVP provided to a population of 10,000 can save the public purse $1.3M over 10 years. Apply this to the 12.9 million Canadians living with heart disease, type 2 diabetes and obesity, the numbers are incredibly impressive!
SAVINGS
When applied to a population of 10,000 patients enrolled in the CHANGE program, My Viva Plan would result in total budgetary savings of $1,303,026 over the 10-year time period.
COST-EFFECTIVENESS
My Viva Plan has a 77% probability of being cost-effective for patients in the CHANGE program compared to the CHANGE program alone as it reduces costs and increases patient health.
